The book Port Cities, Pollution, and Public Health: Environmental Health Issues from Shipping and Ports, edited by Mariantonia Lo Prete and César Ducruet, is an authoritative addition to an emerging field of research. It represents one of the first systematic approaches to map and analyze the complex relationship linking the development of global logistics hubs to the health and ecological costs borne by nearby urban populations.

The work adopts a rigorous transdisciplinary perspective, moving beyond sector boundaries to make disciplines like coastal planning, quantitative geography, economics, public law, epidemiology, and energy engineering actively interact and cooperate. This variety of perspectives also takes on an international dimension, thanks to the contributions of numerous scholars and experts working across five continents.
Together, these contributions prove that decades of progressive physical and functional separation between port areas and historic city centers have not eliminated the negative effects of maritime infrastructure. On the contrary, the impact of industrial terminals and commercial docks continues to spread into and affect the surrounding urban areas. This phenomenon is driven both by heavy truck traffic and the continuous presence of the cruise sector. Both elements are deeply rooted in the heart of densely populated metropolitan areas, with direct consequences for public health.

In the introductory chapter, Mariantonia Lo Prete and César Ducruet define the book’s theoretical framework, combining the analysis of socio-economic systems with the strict study of natural systems. This integrated, systemic approach is built around two key concepts: the idea of the “Social-Ecological System” and the multidisciplinary “One Health” paradigm. The latter, historically taken from medical sciences, is applied here to the port sector in an innovative way to highlight the structural connections linking the health of marine ecosystems to the biological well-being of local communities.
The study highlights the built-in limits of current scientific literature and public policies. It exposes a deep imbalance between the large number of studies dedicated to pollution monitoring techniques and the lack of research focused on the specific, long-term health effects of port activities.
To fix this imbalance, the editors identify four main methodological and practical gaps that the book aims to address. The first critical issue is the difficulty of isolating pollution sources within complex urban areas and port cities. The mixing and overlapping of different pollution sources makes it extremely hard to separate the direct impact of ship emissions from those produced by nearby industrial complexes. Secondly, this issue is tied to the absence of uniform sampling standards: the lack of a unified global model for identifying sources (source-apportionment) prevents a direct comparison of health data between ports in different nations. On the modeling side, the editors point out that traffic from the hinterland is systematically ignored. They show that most traditional predictive models underestimate the real health burden because they exclude emissions from heavy commercial vehicles moving between the port and land transport networks. Finally, the study reveals that the demographic samples used in medical studies so far are incomplete. These studies tend to overlook the dynamic activity profiles of the population and fail to analyze how outdoor pollution penetrates into homes, residential spaces, and workplaces.


The Social Dimension: Risk Mitigation and Perception

The first section of the volume leaves aside purely engineering metrics to explore environmental impact starting from social perception and geographical imbalances. Opening this analysis is the systematic study by César Ducruet and Mariantonia Lo Prete, which maps local European initiatives and reveals a clear contrast: while large Northern European hubs have the resources to launch individual energy transitions, small and medium-sized ports along the Mediterranean are forced to work together through collective partnerships funded by the European Union. This gap clashes with the quantitative data presented by Lorena Garcia-Alonso, who highlights a paradox: the maritime transport sector, despite being efficient over long distances, ranks as the world’s sixth-largest polluter, releasing 4% of its emissions directly on the land side, right next to residential areas.

The distance between institutional planning and the actual experience of local communities emerges clearly in the research conducted by Julia Appercelle, Marie-Laure Baron, Olivier Bueno, Sophie Cros, and Alicia Jacquette in the French cities of Le Havre and Rouen. The study shows that although 60% of residents consider protecting health an absolute priority over dock emissions, as many as 73% are completely unaware of active environmental projects, revealing a serious deficit in public communication. Shifting the focus outside Europe, the essay by Milena Machado, Severine Frère, and Jane Meri Santos on a coastal district in Brazil demonstrates that formal compliance with legal limits does not equal the well-being of the population; they propose a statistical model that uses the sense of “annoyance” perceived by citizens as an indicator for policy decisions. This urgency is equally clear in Hervé Sourya Malazoue’s contribution on the African port of Lomé, Togo, where the lack of regulatory protections and personal protective equipment results in a very high incidence of bronchitis, asthma, and cardiovascular diseases among operators, turning the workforce and residents into collateral victims of the logistics cycle.

The Role of Time: The Impact of Ports on Health in a Historical Perspective

The second part of the volume introduces a necessary historical perspective, demonstrating how the port-city interface has always been a difficult area to manage in terms of vulnerability and biological risk. Mathias Tranchant opens this section by offering a new look at the critical issues affecting around 612 ports on the French Atlantic coast during the Middle Ages. He describes them as fragile ecosystems, constantly exposed to epidemic contamination due to the continuous storage of goods and the movement of people. The evolution of control measures in the 18th century is analyzed by Frédéric Davansant, who deconstructs the famous “Marseille model” based on lazarettos and health passports. The author highlights that at the time, there was no cohesive public health management led by the State; rather, it was a flexible system where quarantine requirements constantly had to compromise with the mercantile elites’ fear of seeing their businesses and trade blocked.

The concrete layout of these isolation places is explored by Sarah E. Hinman, who maps the urban planning dimension of lazarettos in England, Australia, and the United States up to the beginning of the 20th century. Her research shows how these areas of confinement have undergone a process of heritage preservation and historical memory over time, drawing an evocative parallel with the contemporary return of quarantine in the form of self-isolation. Closing the historical section is the essay by Jörg Vögele and Grażyna Liczbińska, focused on European ports between the 19th and early 20th centuries. The authors describe their contradictory nature: while extremely high population density and job insecurity turned them into highly dangerous places that accelerated the spread of diseases, the urgency of managing these biological crises also converted port cities into actual “laboratories” for developing the first public health strategies.

Empirical Methodologies and Quantitative Indicators to Measure the Health Impact of Ports and Maritime Transport

The third section defines the analytical and technical core of the volume, as it offers concrete tools to measure the negative effects produced by the maritime sector. Daniele Contini and Eva Merico open this section by examining the chemical composition of ship emissions. Their research demonstrates that Emission Control Areas (ECAs) have effectively but only partially reduced these effects. The public health impact of these emissions is tested on a European scale in the essay by César Ducruet, Mariantonia Lo Prete, Magali Dumontet, Barbara Polo Martin, Charbel Alkhoury, Ling Sun, and Sheng Zhang, which analyzes 120 urban-port areas between 2000 and 2019. By using models and factor analyses, the study identifies three specific profiles of port cities – critical, tourist, and metropolitan – providing a fundamental classification for those involved in contemporary urban planning and development. This scenario repeats with similar characteristics in China, as described in the chapter by Sheng Zhang, César Ducruet, and Liehui Wang. The authors demonstrate how the political decision to focus solely on commercial scale (scale-first) and large cargo volumes has significantly worsened health conditions in cities. Indeed, the study records a direct correlation between the activity of terminals handling bulk cargo or containers and the incidence of cardiovascular and metabolic diseases among residents.

To address these issues, Charbel Alkhoury proposes an innovative solution: the Port Performance Capability Index (PPCI). Drawing inspiration from Amartya Sen’s economic welfare theories, the author creates a new indicator that goes beyond the rigidity of old economic rules. This tool allows for the evaluation of a port’s efficiency by integrating health, environmental, and social governance costs. An application of this transition is offered at the end of the section by Ling Sun, who examines 35 Chinese port cities over the 2013-2023 decade. Through a statistical model, the study demonstrates that dry bulk cargo causes the greatest biological damage, suggesting a specific industrial policy: shifting coastal ports toward cleaner container flows and restricting liquid bulk cargo to inland river areas.

Institutional Reforms and Governance Models

The final part of the volume shifts its focus toward the regulatory solutions and resilience strategies needed to manage territorial complexity. Claude Comtois, Serge Novikov, and Sarah Lutfy open the section by focusing on the occupational health of seafarers. By studying over 1,813 administrative ship detentions carried out by the Port State Control under the MLC 2006 convention, the research maps the health and sanitation deficiencies of global fleets, proposing automation and digitization of onboard processes to reduce biological risk. The inseparable link between port and city is further explored by Sabah Zrari Djellid through the Chilean cases of Valparaíso and San Antonio, where the author introduces the multidimensional concept of “Quality of Life” (QoL) promoted by UN-Habitat. The essay demonstrates that logistics efficiency does not automatically translate into local well-being, calling for a move away from old post-event (ex-post) mitigation models in favor of preventive urban planning.

This perspective finds an interesting application in the essay by Matteo Nicolini and Giuseppe Rigobello on the Venice Lagoon. Through the study of the industrial hub of Porto Marghera, the authors expose the processes of coastal grabbing carried out by large international corporations and cruise tourism, which have altered ecosystem balances and fueled a “toxic heritage” capable of compromising both the collective memory of places and the health of residents. The legal obstacles preventing real change are analyzed by Marine Chouquet, Mariantonia Lo Prete, and Catherine Roche, who highlight a sharp contradiction in public laws: while recognizing the damage caused by emissions and noise pollution, legal texts deny port authorities actual enforcement and decision-making powers regarding health, subordinating them to other state bodies. The volume closes with the overview offered by Theo Notteboom, Athanasios Pallis, and Patrick Verhoeven on how the global port sector responded to the emergency caused by the COVID-19 pandemic. Through the analysis of seventeen reports from the Port Economic Impact Barometer, the authors demonstrate that the resilience of global supply chains depended on the flexibility of hygiene procedures at port entry points, offering a valuable lesson on structural risk management in an era of global polycrisis.

Conclusions

The strategic value of Port Cities, Pollution, and Public Health lies in its capacity to propose a different perspective compared to a management style often based exclusively on profit, which has guided the planning and development of port waterfronts over the last forty years. In this volume, editors Mariantonia Lo Prete and César Ducruet demonstrate with absolute scientific rigor that the competitiveness of a maritime port can no different be measured apart from its health and social impact on the host city.

Through the integration of mathematical models, sociological surveys on perceived discomfort, historical reviews, and innovative proposals, the work offers a theoretical and practical guide for experts and operators. The text demonstrates that protecting the environment and health by applying the “One Health” paradigm to the port sector does not represent a constraint or a barrier to development; on the contrary, it is the only viable solution to ensure that the public accepts these infrastructures. Given the complex diversity of the international case studies presented, the publication serves as a manual for urban planners, legal experts, port administrators, and institutional representatives, offering the tools to transform the port-city interface from an arena of conflict into a laboratory of shared sustainability and territorial resilience.

He is Geographer and Senior Researcher at the French National Centre for Scientific Research (CNRS), France. He is currently working at the EconomiX laboratory (Paris-Nanterre) on the local impact of contemporary maritime globalization.
